Fill out our New Patient Intake Consent below to become a new patient.
Everyone must fill out the New Patient Paperwork document. Please choose which Vet Referral is needed for your appointment. If you are a new Chiropractic patient, please fill out the Chiropractic Vet Referral document and if you are a new Underwater Treadmill client, please fill out the Underwater Treadmill Vet Referral document. Both forms (New Patient & Vet Referral) are required to be filled out and returned to us by email BEFORE we can set up an appointment with one of our doctors/practitioners.
All dogs must be on a leash at all times when on the premises of The Corporate Court Business Park as well as the inside of the Maryland Animal Chiropractic Center. We ask that you pick up after your pet. We have provided bags and a trash can outside our front door for your convenience.
